Good Faith Estimate for Self-Pay and Out-of-Network Clients

Effective January 1, 2022, Under Section 2799B-6 of the Public Health Service Act, healthcare providers, and healthcare facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” (GFE) of expected charges. The GFE shows the costs of items and services that are reasonably expected for services provided. The estimate is based on information known at the time the estimate was created. It does not take into account any reimbursement that you may receive as a result of out-of-network benefits.

The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. If you are billed for more than this Good Faith Estimate, you have the right to dispute the bill.

You may contact myself, Ashley, to let me know if the billed charges are higher than the Good Faith Estimate. You can ask me to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask about the financial assistance options available.

In general fees for intake, sessions are $100 for adults and $80 for teens and individual therapy sessions are $65 for teens and $80 for adults and can range in frequency from weekly to monthly depending upon your specific needs and goals.

Free 15-minute Therapy Consultations

I am providing free 15-minute consultations. During your consultation, I will discuss with you, the fees associated with the expected services and answer any questions that you may have.

What about Insurance?

At this time, ACT Mindful Therapy does not accept insurance but we hope to accept most major insurance groups soon.

No Show Fees

First Missed Appointment Without Notice: I will send you a reminder of your appointment 15 minutes after the start time. You will not be charged for this missed appointment.

Second Missed Appointment Without Notice: I will send you a reminder of your appointment 15 minutes after the start time. 30 minutes after the start time, you will be charged $50 for the missed session.

Third and Following Missed Appointments Without Notice: I will send you a reminder of your appointment 15 minutes after the start time. 30 minutes after the start time, you will be charged $80 for the missed session.
